Fill out the New Client form once, and the New Patient form for each pet. You can add up to two pets on a single patient form.
If your pet has records from a previous vet, you can upload them directly on the New Patient form as a PDF or ZIP file.
Even if you’ve submitted the forms online, plan to arrive 5–10 minutes before your appointment to get settled in.
